I tend to read 6 on the first Selective Service checklist as being a reference to his father. Since we know from The Winter Soldier that Joe Rogers is buried in the States, and Steve never says he was killed in action, I think it's not unreasonable to think that he died later, in the US, from complications caused by exposure to gas, and several of the items on that list are also associated with exposure to several of the agents used in WWI.
